Concord Park is a monthly rental community offering Independent Living, Assisted Living and state-of-the-art Compass Memory Support. Our supportive services, delivered with care and compassion, enhance residents' independence and dignity and provide peace of mind for residents and their families. We offer a range of accommodations and services to meet individual needs and preferences. Specific monthly rates are based on the apartment type and location, as well as service options chosen by the resident.

• Personalized Wellness Care Plan developed by interdisciplinary team
• Assistance with personal care, including leisurely bathing and dressing

• Family-style dining with meal choices incorporating healthy foods and delicious recipes based on our state-of-the-art Memory Preservation Nutrition® Program
