We often work with local families who move a relative from out of state to one of our communities to be closer to them. We also work with families who live far away yet their relative chooses to move to one of our communities to stay in the town they know and love. In either situation, we coordinate efforts with eldercare professionals and families to ensure a successful move.
Expertise in Financial Programs
SLR Executive Directors are well versed in the programmatic and eligibility features of various financial programs to help with the costs of assisted living and we work closely with referral sources to determine if one of them would benefit a client. Additionally, our on-site staff coordinates with local healthcare providers for Medicare-reimbursed services whenever possible, helping to keep residents' healthcare costs down.

On Thursday at 11 a.m., an SLR Executive Director received a call from a hospital case manager. The case manager had a patient ready to be discharged who did not meet the leveling requirements for a skilled rehab setting, but who would not be safe at home at this time. By 12 p.m. the Executive Director was at the hospital to do an assessment of this individual, and by 1:30 p.m. the involved family member was at the community for a tour. That evening, at 6 p.m., the community moved this woman into a beautifully furnished apartment, complete with toiletries, towels and all of the niceties that make a new space feel comfortable. The community’s nurse coordinated the delivery of the new resident’s medications and oriented the staff to the new resident’s needs. By 10 a.m. the next day, VNA services were in place and the VNA nurse and physical therapist were working with the new resident to address all safety concerns and needed adaptations to the apartment.
