We are Your Care Team Partners

When someone chooses to make SLR Marlboro their home, we become their dedicated partner in care. 

Along with the wellness and support services offered at SLR Marlboro, we work closely with your healthcare providers to gain a complete understanding of your loved one’s care needs. If you’re looking for a provider, we collaborate with a variety of local professionals who offer onsite services for added convenience, and we can refer you to additional practices in Marlboro County. Whether you’re already a resident or transitioning to your new home, we can support all your needs with visiting physicians, onsite mental health support, pharmacy and outside care coordination, and transportation to scheduled medical appointments.

Onsite Care Partners

Primary Care Coordination

For residents who prefer to receive care directly in their apartments, we offer onsite primary care services once a week, partnering with Dr. Atul Bhasin of Atlas Medical Care and Dr. Pass. We also work with Dr. Adil Manzoor, who offers telehealth appointments as well as onsite care weekly or as needed. 

If lab services are required, we work with Premier Medical Lab, who is at the community every Tuesday.

Mental Health & Wellness Services

We know that our residents’ mental health is just as important as their physical health. If you are working through a difficult transition or challenge or just need someone to talk to, these local providers are here to help. We work with: 

 

Direct-to-Community Pharmacy Coordination

With our pharmacy coordination, there’s no need to travel to pick up prescriptions! We work with IPPC Pharmacy, who will deliver resident medications as needed. Residents can also choose to receive extra clinical support through drug reviews.

Visiting Nurse and Speech, Physical & Occupational Therapy Resources 

If you have conditions that require assessment, rehabilitation and recovery, we bring in local practitioners to support you in returning to your day-to-day activities as soon as possible. Residents have the option of working with FOX Rehab and VNA Health Group, and we also bring in home health agency Senior Helpers to provide private care. FOX Rehab has a PT and OT gym at SLR Marlboro, and their team is onsite for rehab services 7 days a week.

Vision, Dental, & Podiatry Providers

If you are seeking specialist care, providers in our network offer additional services such as vision work, dental care, and podiatry.

Radiology & Dermatology Providers

If you are in need of radiology or dermatology care, we welcome Fastrad Mobile Radiology and Onsite Dermatology of New Jersey directly to the community on an as-needed basis.

Hospice Partnerships

We also work with Grace Healthcare Services, who can offer palliative and hospice right in our community 7 days a week. 

Outside Care Coordination

If a resident needs to leave our community for a hospital or rehab stay, we will support you every step of the way. We maintain open communication with residents, families, and providers throughout the process. Our nursing team will maintain an open dialogue with Case Managers in order to ensure a smooth transition back into the community. 

Our shared goal is to bring your loved one back home to SLR Marlboro. Together, we will look over any care plan changes and coordinate any additional services that your loved one may need.
