Alongside the wellness and support services we offer here in the community, we work with your healthcare providers to ensure we have a complete vision of their care needs. If you’re looking for a healthcare provider, we bring in local professionals who can offer convenient onsite services, and we’re happy to refer you to other trusted practices in the Greater Boston area. Whether your loved one is already a resident at Standish Village or has recently joined our community, you can count on us to provide support every step of the way, including visiting physicians, onsite mental health services, pharmacy coordination, and transportation to medical appointments nearby.
Resident choice is at the heart of our care coordination. You always have the option to continue working with current providers you know and trust, or to work with our onsite care partners– who you choose as part of your healthcare network is entirely up to you.

Primary Care Coordination
For residents who would like to receive onsite care in the comfort of their apartments, we collaborate with a range of primary care physicians, nurse practitioners, and physician assistants to bring you top-notch care. Our partners include:
- Vesta Healthcare, providing 24/7 support, onsite monthly or as needed
- BU Geriatrics, onsite quarterly
- Gather Health, onsite by appointment
- Harbor Health (PACE), onsite by appointment

Mental Health and Wellness Services
If you need support to work through life’s challenges, our trusted collaboration with behavioral health providers means that you have the resources you need along the way. We work with Supportive Care Behavioral Health to bring in a Psychiatric Nurse Practitioner. With this partnership, residents have:
- Onsite community visits with professionals licensed in behavioral health, talk therapy, psychology and geripsych
- Services covered by most insurance plans, including Medicare, with no referral necessary
- Support when you need it most, including periods of loss and grief, social isolation and loneliness, depression and anxiety, and more
- Resources for residents experiencing changes in medical/health condition or behavior
- Residents also make use of this service as a support system while facing symptoms of cognitive decline, dementia, and memory loss
Direct-to-Community Pharmacy Coordination
With Greater Boston Long Term Care Pharmacy, residents and families no longer have to travel to pick up prescriptions! GBLTC Pharmacy visits the community as needed for medication deliveries or medication changes, and residents can also choose to receive extra clinical support through drug reviews and assessments.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you have conditions that require evaluation, rehabilitation, and recovery and prefer to receive the care within the community, we collaborate with local providers to help you get back to your normal activities as soon as possible. The following providers offer support at Standish Village:
Dental & Podiatry Providers
For residents seeking specialist care, we work with dental and podiatry providers in the area, including Cummins Family Dental and Elite Feet Podiatry (onsite monthly).
Outside Care Coordination
If your loved one needs to leave Standish Village to receive care at a hospital or rehab, we will be there to support you throughout the treatment process. We will stay in touch with residents, families, and the provider at every stage, and our nursing gteam will also communicate with Case Managers to make for a smooth transition back to the community.
Our shared goal is to help bring your loved one back home to Standish Village. We will work together to review any changes to their care plan and arrange for additional services to ensure they have the support they need.
