When someone joins the Heights Crossing community, we become their partner in healthcare.
Alongside the wellness and support services we offer, we also coordinate with your healthcare providers to gain a deep understanding of your care needs. If you are seeking a healthcare provider, we collaborate with local practices who can bring their services directly to the community, or we are happy to connect you to other providers in the greater Brockton area. Whether you are already a resident at Heights Crossing or preparing to move in, you can feel comfortable knowing we provide support with visiting physicians, onsite mental health services, pharmacy coordination, and transportation to medical appointments in the area.
At Heights Crossing, resident choice is central to everything we do. You can always choose to continue working with the current healthcare providers you trust, or you are welcome to work with our onsite partners– the choice is entirely yours.

Onsite Care Partners
Primary Care Coordination
We offer onsite primary care services on an as-needed basis, bringing Dr. Allen Waltman into our Compass Memory Support Neighborhood.
Mental Health & Wellness Services
We know that our residents’ mental health is just as important as their physical health. If you need someone to talk to, we welcome Pinnacle Health Management into the community. Pinnacle has a highly skilled team of Clinical Nurse Specialists, Nurse Practitioners, Psychologists, and Licensed Clinical Social Workers focused on reliable, diligent, and person-centered care.
Direct-to-Community Pharmacy Coordination
With our pharmacy partnership, there’s no need to travel to pick up prescriptions! We work with Greater Boston Long Term Care Pharmacy, who will deliver resident medications as needed. Residents also have the option for additional clinical support with comprehensive drug reviews and assessments if you prefer.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you need help with conditions that require assessment, rehabilitation and recovery, we bring in local practices so that you can return to your daily activities as soon as possible. The following offer onsite support at Heights Crossing:

- By Your Side Nursing Care, for residents seeking private care options
Dental & Podiatry Providers
Residents seeking specialist care have the option to work with these local providers, providing dental and podiatry services.
Practices we work with, by appointment or onsite as needed:
- Mobile Dental Hygiene Services, at the community every 6 months
- Dr. Richard Hacker D.P.M Podiatry, at the community every 10 weeks
Hospice Partnerships
We work with Beacon Hospice, Tides Hospice, and Autumn Hospice, who can offer palliative and hospice care right in our community.
Outside Care Coordination
If your loved one needs to leave Heights Crossing for additional care, you can count on us to be by your side throughout the process. We’ll maintain regular communication with you and the hospital or rehab center, ensuring a smooth transition back to our community. Our nursing team will collaborate with Case Managers and assist with discharge planning to provide seamless care continuity.
It is our shared goal to help your loved one get back home to Heights Crossing. We will collaborate with providers to make any necessary changes to their care plan and arrange any extra support they might need to make the transition as smooth as possible.
