At Heights Crossing, we are eager to collaborate with each resident’s healthcare team to understand their health, care needs, and treatment plans. Alongside our network of trusted local specialists, many of whom offer services right here in our assisted living and memory care community, we make sure residents receive the support they need. When residents require additional care beyond what we can offer, we refer residents to the right professionals to ensure their physical and mental well-being. We also offer transportation to local medical appointments.
Whether you are already a resident at Heights Crossing or are transitioning to your new home, the care you need is right around the corner, from in-house primary care services to strong partnerships with rehab centers, dental and vision providers, psych and geripsych, and more!
Primary Care Coordination
Physicians, Lab Services
We offer onsite primary care services every week, bringing Dr. Allen Waltman into the community.
If lab services are required, we work with Quest Lab.
Mental Health & Wellness Services

We know that our residents’ mental health is just as important as their physical health. Whether you are working through a difficult transition or just need someone to talk to, we welcome Pinnacle Health Management into the community. Pinnacle has a highly skilled team of Clinical Nurse Specialists, Nurse Practitioners, Psychologists, and Licensed Clinical Social Workers focused on reliable, diligent, and person-centered care.
Pharmacy Coordination
With our pharmacy partnership, there’s no need to travel to pick up prescriptions! We work with Greater Boston Long Term Care Pharmacy, who delivers resident medications as needed. With this partnership, residents have access to:
- Free medication delivery
- Comprehensive drug review by a pharmacist
- 24-hour services
- Pharmacy participates in resident assessment process
- Access to complete clinical support
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
We partner with top-notch visiting nurse associations to help you with conditions that require assessment, rehabilitation and recovery so that you can return to your daily activities as soon as possible. We work with:
- Caretenders (VNA, PT, OT, ST)
- Brockton VNA
Caretenders has a unique person-centered, quality-focused, and community-minded approach to ensure compassionate care. The Caretenders team takes the time to understand the specific needs of each individual to ensure the best possible treatment.
Vision, Dental & Podiatry Providers
Other specialists in our network provide additional services such as vision and dental care and podiatry, some of which are offered onsite at our community.

Hospital, Rehab & Hospice Partnerships
If a resident needs to leave our community for a hospital or rehab stay, our nursing team supports residents and families every step of the way so that we can welcome residents back when they are ready. We recommend:
