If you are seeking a healthcare provider, we partner with several area professionals who offer onsite services. We are happy to refer you to these trusted practices in the greater Bourne area. Whether you are already a resident at CCSR or are transitioning to your new home, you can rest assured knowing that we’ve got you covered, with visiting physicians and nurses, mental health services, and pharmacy coordination.

Onsite Care Partners
Primary Care: Physicians, Lab Services, Visiting Nurses
For those seeking onsite primary care, we work with Dr. Michol Negron, who is at the community monthly and available via telehealth as needed.
We also bring in Quest Diagnostics, who provide onsite blood work/lab draws for residents 2-3 times per week.
Finally, we work with Bayada Home Health Care and VNA of Cape Cod as our Visiting Nurse Associations. Caregivers offering skilled nursing services, on-site PT, OT, speech therapy, and more come directly to the community as needed.
Mental Health and Wellness Services
When you need extra support to navigate transitions and challenges in life, our partnership with cognitive/behavioral health service providers ensures that you have someone with you every step of the way. We bring in a team of Licensed Mental Health Counselors, Psychologists, and Licensed Clinical Social Workers to provide care.

Direct-to-Community Pharmacy Coordination
Your days of having to travel to pick up prescriptions are over! The North Falmouth Pharmacy team is onsite daily to deliver and package medications.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
We work with Bayada Home Health Care and VNA of Cape Cod as our Visiting Nurse Associations. Caregivers offering skilled nursing services, on-site PT, OT, speech therapy, and more come directly to the community as needed. These providers will help you with conditions that require assessment, rehabilitation and recovery so that you can return to your normal activities as soon as possible.

Outside Care Coordination
When a resident needs to leave the community for additional care, we keep in touch with you and the provider throughout the entire process. Our nursing team will stay in touch with Case Managers and be involved in the discharge planning process.
Hospitals and Inpatient Rehabs: When a resident needs to go to the hospital or a rehab center outside of the community, our main goal is to help your loved one return home to CCSR. We will review any necessary care plan changes and explore additional services that we can introduce to provide them with the support they need.
