At Cornerstone at Canton, we are eager to work with you and your healthcare providers to provide you with the highest quality of care across a variety of specialties. Cornerstone is truly a hub of resident care, and we can bring a range of outside services right to our assisted living and memory support community. We connect you with the healthcare professionals you need to maintain your holistic health. We also offer transportation to scheduled medical appointments in the greater Canton area.
Whether you are already a resident at Cornerstone or are transitioning to assisted living, you can feel comfortable knowing the care you need is right around the corner. We provide in-house primary care services and have strong partnerships with local care providers like PCPs, physical and occupational therapists, geriatric care managers, dental/vision providers and more.
Primary Care Coordination
Physicians, Geriatric Care, Lab Services, Visiting Nurses
We connect you to highly skilled local primary care providers, and we are happy to take you to your scheduled appointments in our community transportation. We work with Pinnacle Home Healthcare Services, whose team is made up of Dr. Maria Connor and Avril Jackson-Black, NP. The Pinnacle team visits each resident once per month, with additional visits as needed. Cornerstone and Pinnacle are in constant communication to ensure that both teams are up to date on resident needs.
For those seeking geriatric care managers, we partner with Elise Cedrone, who provides eldercare consulting, care coordination, and additional support.
Cornerstone also works with Caretenders Home Care and Quest Diagnostics Home Draw for bloodwork and additional lab services.
Mental Health and Wellness Services
Talk Therapy, Neuropsychology
When you need extra support to navigate challenges and changes in life, our partnership with behavioral health service providers ensures that you have someone ready to support you. We collaborate with LifeStance Health to bring in a team of psychiatrists, psychologists, and licensed therapists.
With this partnership, residents have:
- Access to highly trained clinicians via onsite community visits or telehealth appointments
- A personalized care plan tailored to best serve you
- Services covered by most insurance plans, including Medicare, with no referral necessary
- Support for the many challenges life can bring, including difficult transitions, anxiety and depression, withdrawal from social activities, existing mental health concerns, loss and grief, and more
- Support for residents experiencing a medical or health condition change or behavioral changes
- Residents also utilize this service as an extra source of support to help them manage symptoms of cognitive decline and dementia or memory loss
For additional support, we work with Liz Gruber, NP who is onsite once a month or as needed to provide medication oversight and manage any cognitive, behavioral, and psychological needs.
Pharmacy Coordination
We work with Greater Boston Long Term Care Pharmacy to provide traditional pharmacy care and we have the ability to offer one-on-one counseling and resident education without residents having to leave the community. Benefits of this partnership include:

- Comprehensive drug review by a pharmacist
- 24-hour services
- Pharmacy participates in resident assessment process
- Access to complete clinical support
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
We can recommend a wide range of excellent home health agencies to help you with conditions that require assessment, rehabilitation and recovery so that you can return to your normal activities as soon as possible. Some of these providers will even see residents directly in our community.

Vision, Dental, Hearing & Podiatry Providers

Hospital, Rehab & Hospice Partnerships

When a resident needs to go to the hospital or rehab, we can refer families to trusted providers in the area. We keep in touch with residents throughout this entire process, even when they are not in our community, because we want to be able to welcome them back when they’re ready.
