We are Your Care Team Partners

When someone makes Cornerstone at Canton their home, we become their partner in healthcare.

We coordinate the wellness and support services residents receive at our community with their healthcare providers to gain a well-rounded vision of their care needs. If you are seeking a healthcare provider, we have a range of local providers who offer convenient onsite services, or you can choose from a number of other practices in the greater Canton area. Whether you are already a resident at Cornerstone or are transitioning to your new home, you can feel comfortable knowing that you have a variety of onsite and outside care options for any services you need. 

Resident choice is at the heart of our approach, and your healthcare provider network is completely in your hands. When you join the Cornerstone community, you can continue working with the providers you feel most comfortable with and maintain existing relationships, or you can choose to work with our onsite providers.

Onsite Care Partners

Primary Care Coordination

For those who prefer the convenience of receiving care directly in the community, we bring in Pinnacle Home Healthcare Services. The Pinnacle team visits each resident once per month, with additional visits as needed. Cornerstone and Pinnacle are in constant communication to ensure that both teams are up to date on resident needs.

While we primarily refer to Caretenders Home Care as our Visiting Nurse Association, you always have the option to work with any VNA agency you choose.

Mental Health and Wellness Services

If you need extra support to navigate challenges and changes in life, our partnership with behavioral health service providers ensures that you have someone ready to support you. 

We work with Liz Gruber, NP, who is onsite regularly to provide medication oversight and manage any cognitive, behavioral, and psychological needs.

Direct-to-Community Pharmacy Coordination

Our partnership with Greater Boston Long Term Care Pharmacy ensures that you don’t have to worry about traveling to pick up your prescriptions! Residents receive regular medication deliveries and have access to extra clinical support through drug reviews and assessments if you choose.

Speech, Physical & Occupational Therapy Resources 

If you have conditions that require assessment and rehabilitation and would like to receive care directly in the community, we bring in local assistance to help you return to your normal activities as soon as possible. Residents have the option of working with our onsite providers for convenience. Caretenders VNA is onsite weekly or as needed for Physical Therapy, Occupational Therapy, and Speech Therapy services. A Step Up Physical Therapy and Connections Physical Therapy are additional options that use Medicare Part B to allow for long-term therapy.

Vision, Dental, Hearing & Podiatry Providers

Other specialists in our network of care provide additional services such as dental care and podiatry, some of which are offered onsite at our community.

Hospice Partnerships

We refer to many hospice practices, including Old Colony Hospice.

Outside Care Coordination

If your loved one needs to receive additional care outside of the community, we will support you every step of the way because we want to be able to welcome them back to Cornerstone when they are ready. We will regularly maintain contact between residents, families, and the hospital or rehab center throughout their stay. Our Wellness team will also work with Case Managers and play an active role in planning for your loved one’s return. 

Our main goal is to get your loved one back home to Cornerstone. Together, we will review any necessary care plan changes and arrange for additional services to support them.
