At Cornerstone at Milford, we are happy to work with you and your healthcare providers to recommend the best possible care across a variety of specialties. We can bring several additional services right to our assisted living and memory support community, connecting you with the professionals you need to maintain your mental and physical health. We also offer transportation to scheduled medical appointments in the area. 

Whether you are already a resident at Cornerstone or are transitioning to your new home, we have your back. From in-house primary care services to strong partnerships with local care providers like PT/OT, vision, dental, mental health counselors, we are truly a hub of resident care!

Primary Care Coordination

Physicians, NPs, GCMs, Lab Services

We work with Vantage Healthcare to offer onsite primary care, bringing top-notch health professionals right to your apartment! Vantage’s team includes an onsite Nurse Practitioner (NP) and a remote Nurse Care Coordinator (RN). Benefits of this partnership include:

  • Primary care services onsite once a week at the community
  • Services are often covered by Medicare
  • Coordination of other health care services such as specialists, PT, OT and speech therapy providers.

Our contact at Vantage is Family Nurse Practitioner Karen Magit, RN, who is onsite at Cornerstone every Monday. 

For residents seeking a Geriatric Care Coordinator, we recommend Aging Well Advocates and The Aging Space, who provide onsite eldercare consulting, recommend care options, and offer emotional and family support as needed. 

We partner with Quest Diagnostics for lab draws. 

 

Mental Health & Wellness Services

Talk Therapy, Cognitive Health

When you need extra support to work through challenges and changes in life, our partnership with behavioral health service providers ensures that you are not alone. We collaborate with Vantage Healthcare to bring in Marielle Pitts, Psychiatric Nurse Practitioner (PMHNP) so that residents will always have someone to talk to during difficult times. 

With this partnership, residents have:

  • Access to licensed professionals via onsite community visits or telehealth appointments
  • Support for the many challenges life can bring, including difficult transitions, anxiety and depression, withdrawal from social activities, existing mental health concerns, loss and grief, and more
  • Residents also utilize this service as an extra source of support to help them manage symptoms of cognitive decline and dementia or memory loss

Pharmacy Coordination

We work with Greater Boston Long Term Care Pharmacy and Milford Central Pharmacy to provide traditional pharmacy care and medication delivery. Benefits of this partnership include:

  • Medication delivery directly to apartments
  • Comprehensive drug review by a pharmacist
  • 24-hour services
  • Access to complete clinical support

 

Visiting Nurse and Speech, Physical & Occupational Therapy Resources 

We partner with Caretenders to help you with conditions that require assessment, rehabilitation and recovery so that you can return to your normal activities as soon as possible. Some of these providers will even see residents directly in our community.

Vision, Dental, Hearing & Podiatry Providers

Hospital, Rehab & Hospice Partnerships

When a resident needs to go to the hospital or rehab, we can refer families to trusted providers in the area. We keep in touch with residents throughout this entire process, even when they are not in our community, because we want to be able to welcome them back when they’re ready. 

We refer to the following providers for rehabilitation services:

We also work with the following providers for onsite palliative, hospice, and bereavement support:
