We are committed to supporting your loved one and maintaining a well-rounded vision of their care needs. Along with our in-community wellness and support services, we work closely with healthcare providers to provide any additional services you may need. If you are searching for a provider, we collaborate with trusted local professionals who can offer services right here in our community, and we’re happy to connect you with other practices in the Greater Farmington area. Whether you’re already a resident or just settling into your new home at Farmington Station, you can rely on us to provide convenient access to visiting physicians, mental health services, pharmacy coordination, and transportation to appointments.

Resident choice matters the most in care coordination. We encourage you to work with the healthcare providers you trust and feel most comfortable with, whether they are your current providers or our onsite partners.

Onsite Care Partners

Primary Care Coordination

For residents who prefer to receive care in the comfort of their apartments, we partner with Reliant Housecalls to offer primary care services at our community every Tuesday and Friday. 

Those seeking a Geriatric Care Manager have the option of working with Lisa Cheney at Aging Well NWCT. 

We work with Cirrus Labs, who can come straight to Farmington Station. For convenience, our team of nurses will handle orders whenever possible.

Mental Health & Wellness Services

We know that our residents’ mental wellbeing is just as important as their physical health. If you need extra support to work through challenges and changes in life, you can choose to work with Dr. Joanna Fogg at Hartford Hospital, who specializes in treating older adults. Dr. Fogg sees residents at the community on a monthly basis.

Direct-to-Community Pharmacy Coordination

Your days of having to travel to pick up prescriptions are over! We work with Suburban Long Term Care Pharmacy and Beacon Prescriptions, who will deliver resident medications twice daily. Forms are available to take advantage of this convenient partnership and service.

Visiting Nurse and Speech, Physical & Occupational Therapy Resources 

Our partnership with FOX Rehab is an option for residents with conditions that require assessment and rehabilitation and would like to be seen directly in the community.

Vision, Dental & Podiatry Providers

Residents seeking specialists in our network of care have access to additional services such as dental care and podiatry, some of which are offered onsite at our community.

Hospice Partnerships

We work with Brighton Hospice, Compassus Hospice, and McLean Homecare & Hospice, who can offer palliative and hospice right in our community.

Outside Care Coordination

If a resident needs to leave our community for a hospital or rehab stay, our nursing team stays in touch every step of the way to make sure we are ready for their return. 

Our shared goal is to help your loved one return home to Farmington Station. Together, we’ll review any needed updates to their care plan and explore additional services to ensure they have the support they need.
