Along with the wellness and support services your loved one receives at our community, we collaborate with their healthcare providers to gain a comprehensive vision of their care needs. If you’re looking for a healthcare provider, we partner with a variety of local professionals who can offer onsite services for convenience, and we can also connect you with other practices in the greater Ayer area. Whether you’re already a resident at Nashoba Park or have just joined our community, you can feel confident knowing we’re here to support you with visiting physicians, onsite mental health services, pharmacy coordination, and transportation to scheduled medical appointments in the area.
Resident choice is the most important aspect of our approach. You always have the option to work with the healthcare providers you feel comfortable with, whether they are your current providers or our onsite partners.
Onsite Care Partners
If you are seeking onsite primary care in the comfort of your apartment, we work with Vantage Healthcare to offer onsite primary care and lab
draws on a biweekly basis. The Vantage team can also coordinate other specialist services, PT, OT, and speech therapy providers for you if you choose.
Direct-to-Community Pharmacy Coordination
No more traveling to pick up your prescriptions– we work with Greater Boston Long Term Care Pharmacy to provide traditional pharmacy care and medication delivery! Residents also have the option to receive comprehensive drug reviews and assessments if they choose.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you have conditions requiring assessment, rehabilitation, and recovery and would like to receive care directly in the community, our onsite partners, VNA Care Network and Caretenders, can help you return to your normal activities as soon as possible.
Vision, Dental & Podiatry Providers
Those searching for specialized services can choose to work with other providers in our network of care, who provide dental care, vision, and podiatry. Some of these services are offered onsite at Nashoba Park.

We bring in Brookhaven Hospice, Ascend Hospice, and Encompass Health for onsite palliative, hospice, and bereavement support.
Outside Care Coordination
If your loved one needs to leave Nashoba Park for additional care, we will be with you throughout the entire process, maintaining open communication between residents, families, and the hospital or rehab every step of the way. Our nursing team will consistently communicate with Case managers and be involved in the discharge planning process.
Our shared goal is to help your loved one return home to Nashoba Park. Together, we’ll review any necessary changes to their care plan and identify additional services that can be put in place to provide the support they need.
