When someone makes Armbrook Village their home, we become their partner in healthcare.
In addition to the wellness and support services that your loved one receives at our community, we coordinate with your healthcare providers to maintain a full picture of their care needs. If you are in need of a healthcare provider, we do bring in a number of local providers who can offer their services onsite for convenience, and we can refer you to other practices in the Greater Westfield Area. Whether you are already a resident at Armbrook or are transitioning to your new home, you can feel comfortable knowing we’ve got you covered, with visiting physicians, onsite mental health services, pharmacy coordination, and transportation to scheduled medical appointments in the area.
Resident choice is at the heart of our approach. Who you choose in your healthcare provider network is completely up to you. When you make the move to Armbrook, you can continue working with the providers you feel most comfortable with and maintain existing relationships, or you can choose to work with our onsite partners.
Onsite Care Partners
Primary Care: Physicians, Geriatric Care, Lab Services
For residents who prefer onsite care in the comfort of their apartments, we have practitioners from AM Medical Primary Care onsite 2-3 times a week offering primary and geriatric care.
For lab services, Labcorp is onsite as needed.
Direct-to-Community Pharmacy Coordination
Through our partnership with Greater Boston Long Term Care Pharmacy, you no longer have to travel to pick up your prescriptions! We work with the pharmacy for free medication deliveries, and residents also have the option for extra clinical support through comprehensive drug reviews and assessments if you choose.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you have conditions that require assessment and rehabilitation and would like to receive care directly in the community, we bring in local assistance to help you return to your normal activities as soon as possible. The following providers can offer onsite support at Armbrook Village:

Residents looking for specialist care can use the services of these local providers, who are in our community regularly.

Hospice Partnerships
We have relationships with Brookhaven Hospice and Baystate Hospice, who can provide in-community care.
Outside Care Coordination
If your loved one does need to leave the community for additional care, you can count on us to stay with you every step of the way. We will be in constant communication with you and the hospital/rehab throughout your treatment. Our nursing team will stay in touch with Case Managers and be involved in the discharge planning process.
We all share the goal of getting your loved one back home to Armbrook, and together we will look at any necessary care plan changes and additional services that can be brought in to support your loved one.
Hospitals and Inpatient Rehabs: When a resident needs to go to the hospital or rehab center outside of the community, we keep in touch with them throughout this entire process, as we want to be able to welcome them back to Armbrook Village as soon as they are ready.
