Alongside the wellness and support services your loved one receives in our community, we work with healthcare providers to gain a well-rounded vision of their care needs. If you’re looking for a provider, we bring in several local providers for convenient onsite care, and we’re happy to refer you to practices in the Greater Cambridge area. Whether your loved one is already a resident or settling into their new home, you can trust us to provide seamless support, including visiting physicians, onsite mental health services, pharmacy coordination, and transportation to scheduled medical appointments.
Resident choice is at the heart of our care coordination approach. When building your provider network, you are free to work with the healthcare providers you know and trust or explore partnerships with our onsite providers—it’s entirely up to you.

Primary Care Coordination
If you are searching for a primary care provider, we can connect you to these local practices, and we are happy to take you to your scheduled appointments in our community transportation.

For those seeking geriatric care managers, we partner with Your Elder Experts, Senior Support Solutions, and Life Care Advocates for all your needs. Residents can also get blood work done at Mount Auburn Hospital, which is right next door to our community.
Direct-to-Community Pharmacy Coordination
We work with Greater Boston Long Term Care Pharmacy to provide traditional pharmacy care so that you no longer have to travel to pick up prescriptions! Residents also have the option of getting extra clinical support with comprehensive drug reviews.
Visiting Nursing, Speech, Physical & Occupational Therapy Resources
If you need onsite support for conditions requiring assessment and rehabilitation, we partner with local providers to help you return to your normal activities as quickly as possible. Our onsite partners include:

We partner with Good Shepherd Hospice for onsite support, and we also bring in Beth Israel Lahey Health at Home and Care Dimensions Hospice as needed.
Outside Care Coordination
If your loved one needs to leave The Cambridge Homes to receive outside care at a hospital or rehab center, you can rely on us for support throughout the entire process. We will maintain consistent contact with residents, families, and providers every step of the way, because we want to be able to welcome residents back to The Cambridge Homes when they’re ready. Our nursing team will also communicate with Case Managers to ensure a smooth transition back to the community.
Our top priority is helping your loved one return home to the community. Together, we’ll evaluate any changes to their care plan and bring in any additional services your loved one may need.
