When someone makes Concord Park their home, we become their partner in healthcare.
In addition to the wellness and support services residents receive at our community, we also maintain a dialogue with your healthcare providers to gain a deep understanding of your care needs. If you are looking for additional healthcare services, we bring in a range of local practitioners who treat residents onsite. We can also refer you to other practices in the greater Concord area. Whether you are already a resident at Concord Park or are transitioning to your new home, we can support your care needs with visiting physicians, pharmacy coordination, PT/OT, onsite dental, and more.
Resident choice is the foundation of our approach, and your healthcare provider network is completely up to you. When you join the Concord Park community, you can maintain existing relationships with your current providers, or you have the option of working with our onsite partners.

Onsite Care Partners
Primary Care Coordination
For residents who want convenient care in the comfort of their apartments, we bring in practitioners from Vantage Healthcare twice a week. With Vantage, residents have access to healthcare professionals with extensive experience in senior care. Vantage’s team includes an onsite Primary Care Physician, Nurse Practitioner (NP) and a remote Nurse Care Coordinator (RN).
Those looking for a Geriatric Care Manager have the option to see Lori Kalinoski, who provides eldercare consulting, coordination and support as needed.
We also work with Emerson Health Lab, whose practitioners are onsite each Wednesday & Thursday for lab services.
Mental Health & Wellness Services
Talk Therapy, Geripsych & Social Work
If you need extra support to work through challenges and changes in life, our partnership with behavioral health service providers ensures that you have someone to talk to. We collaborate with Vantage Healthcare to bring in a team of psychiatrists specializing in senior care.
With this partnership, residents have:
- Access to licensed professionals via onsite community visits or telehealth appointments
- Support for the many challenges life can bring, including difficult transitions, anxiety and depression, withdrawal from social activities, existing mental health concerns, loss and grief, and more.
- Residents also utilize this service as an extra source of support to help them manage symptoms of cognitive decline and dementia or memory loss
Becky Lopresti, SW, our in-house Social Worker, is another readily available resource for residents if they choose.
If you already have a therapist or another mental health professional you’d like to continue seeing, we can arrange transportation for in-person appointments in the area or help you access telehealth sessions.
Direct-to-Community Pharmacy Coordination
We work with Acton Pharmacy, who provide traditional pharmacy services such as medication packaging and delivery directly to Concord Park. With this partnership, you no longer have to travel to pick up prescriptions and can enjoy the worry-free process of getting them delivered right to your door.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you have conditions that require evaluation and rehabilitation and prefer to receive care right here in the community, we partner with local providers to help you get back to your normal activities as quickly as possible.
We most often refer to Bayada Home Health Care, whose practitioners coordinate a variety of rehab services for residents.

Other specialists in our network of care provide additional services such as dental care and hearing, some of which are offered onsite at our community.

Hospice Partnerships
We work with Care Dimensions and Ascend Hospice for onsite palliative and hospice care.
Outside Care Coordination
If a resident needs to leave the community to go to the hospital or rehab, we keep in touch with residents, families, and providers throughout this entire process. We maintain constant communication even when your loved one is not in our community, because we want to be able to welcome them back to Concord Park when they’re ready. Our nursing team will also stay in touch with Case Managers and be actively involved in the discharge planning process.
We all share the goal of helping your loved one return home to Concord Park, and we will review any changes to their care plan and arrange for additional services to ensure they have the support they need.
