We work with Vantage Healthcare and Lincoln Physicians to offer onsite primary care, bringing a highly skilled team right to Concord Park! With Vantage, residents have access to healthcare professionals with extensive experience in senior care. Vantage’s team includes an onsite Primary Care Physician, Nurse Practitioner (NP) and a remote Nurse Care Coordinator (RN). Benefits of this partnership include:

• Primary care services onsite twice a week at the community
• Services are often covered by Medicare
• Coordination of other health care services such as specialists, PT, OT and speech therapy providers

For residents looking for a Geriatric Care Manager, we refer to Lori Kalinoski, who provides eldercare consulting, coordination and support as needed. 

We also work with Emerson Health Lab, whose caregivers are onsite each Wednesday & Thursday for lab draws, x-rays, and urinalysis. 

Talk Therapy, Geripsych & Social Work

When you need extra support to work through challenges and changes in life, our partnership with behavioral health service providers ensures that you have someone to talk to. We collaborate with Vantage Healthcare to bring in a team of psychiatrists specializing in senior care. 

With this partnership, residents have:

• Access to licensed professionals via onsite community visits or telehealth appointments
• Support for the many challenges life can bring, including difficult transitions, anxiety and depression, withdrawal from social activities, existing mental health concerns, loss and grief, and more.
• Residents also utilize this service as an extra source of support to help them manage symptoms of cognitive decline and dementia or memory loss

We also collaborate with Becky Lopresti, SW, our in-house Social Worker who is another readily available resource for residents. 

We work with Acton Pharmacy, who provide traditional pharmacy services such as medication packaging and delivery directly to Concord Park. With this partnership, residents have the worry-free and convenient process of getting their medications on time, right to their doorsteps. 

We can recommend a range of reputable visiting nursing associations and home health agencies to help you with conditions that require assessment, rehabilitation and recovery so that you can return to your normal activities as soon as possible. Some of these providers will even see residents directly in our community.

Other specialists in our network of care provide additional services such as dental care and podiatry, some of which are offered onsite at our community. Residents have access to the essential, top-notch care they need from the comfort of the community or right nearby.

Hospital, Rehabilitation, Skilled Nursing, Hospice, Palliative Care

When a resident needs to go to the hospital or rehab, we can refer families to trusted providers in the area. We keep in touch with residents throughout this entire process, even when they are not in our community, because we want to be able to welcome them back when they’re ready. 

We refer to the following providers for rehabilitation services:

We also work with the following providers for onsite palliative, hospice, and bereavement support:
