WHAT WILL YOU BE DOING?
  • Spending quality time with seniors and providing direct personal care
  • Joining or leading fun activities of your choice with the residents

  • Light housekeeping services, with the support of the housekeeping team
  • Assistance with dining services, as needed
REQUIREMENTS:
  • Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA), strongly preferred
  • Desire to work with seniors, required; Experience preferred but all necessary training is provided for associates
  • Compassionate, patient, committed to teamwork, dedicated, trustworthy, positive attitude

WHAT WILL YOU BE DOING?
The Resident Care Director manages, directs and develops a comprehensive care program for all residents of the community, including traditional Assisted Living residents as well as those in our specialized Alzheimer’s and Dementia care neighborhood. The RCD oversees the training and managing of our team of CNAs and serves as a liaison with family members, community agencies, physicians and care providers.

* This position requires on-call responsibilities to best support the staff and community.

REQUIREMENTS / WHAT WE EXPECT:
A passion for working with seniors. A collaborative mindset that values teamwork. Excellent communication skills. A tech-forward approach to daily operations, especially in this “new normal”. An ability to maintain professional integrity and to build trust, especially in challenging situations. 

This position requires an LPN or RN who is a graduate of an approved nursing program with valid Massachusetts nursing license or otherwise meets the requirements of Assisted Living regulations, or equivalent. Experience in a senior community environment and a means of transportation to complete assessments for new and returning residents is a MUST!

  • Provide support with photography, the creation of a monthly newsletter, updating the community Facebook page, and public relations efforts.

REQUIREMENTS:
  • Experience in event planning, coaching, or facilitation.
  • Associate’s or Bachelor’s degree, preferred.
  • 1 to 2 years experience working with the elderly and/or Alzheimer’s populations; experience providing programs or services to the elderly is preferred.
  • Must be responsible, organized, and dependable with the ability to communicate effectively with senior citizens and their families.
  • Computer skills required.
  • Must have patience, tact, cheerful disposition and enthusiasm.
  • Must have a history of responsible, accident-free driving.

Concord Park is seeking a dynamic, innovative programmer who is creative, friendly, energetic and organized to assist the Program Director in creating, scheduling and implementing programs for our residents. The role is a part time position at Concord Park in Concord, MA. 
 
WHAT WILL YOU BE DOING?
  • The Programming Assistant provides assistance in managing a comprehensive program for active independent seniors, as well as individuals with memory impairments, with the goal of enhancing their social, intellectual, creative, spiritual and physical well­ being. As assigned, instructs and supervises other assistants, Resident Care Associates (RCAs) and volunteers in their participation in activity programming; and serves as a liaison with family members, the larger community and care providers.
  • The Programming Assistant may assist with developing, scheduling and directing these programs daily, including driving residents to off-site activities in the community van.
  • Duties also include providing support for photography, the creation of a monthly newsletter, updating the community Facebook page, and assisting with public relations efforts.

Concord Park’s Social Worker provides comprehensive social services to meet the emotional and social needs of residents at the time of move-in, during their residence, and if necessary when they depart the community. The Social Worker acts as a referral source directing participants and their families to community social service providers, and serves as a liaison with family members, community agencies, physicians, and care providers. This individual administers state Medicaid programs and must be familiar with additional programs for seniors, such as Medicare and the Veteran’s Aid and Attendance Benefit.
 
Available Hours
Part Time, 20 hour position with flexible schedule
 
REQUIREMENTS:
  • Graduate of an accredited college with a degree in Social Service
  • Valid license as a LSW, LCSW, LICSW or LSWA
  • Experience working with elderly clients
  • Must understand and follow the rules and regulations governing Assisted Living communities and various financial programs such as state Medicaid programs

  • Responsibilities include dish washing, heavy cleaning of kitchen counters & storeroom, mopping floors, emptying trash, and other duties as assigned by supervisor.
  • The utility person will work with volunteers, residents of the community and other staff members to complete tasks.

REQUIREMENTS:
  • Must be able to communicate clearly, listen to resident needs and work with both small and large groups.
  • Have excellent problem-solving skills and a strong work ethic.
  • Be able to lift 60 pounds and operate equipment safely.

WHAT WILL YOU BE DOING?

The Wellness Coordinator is responsible for managing the Resident Care Department schedule, specifically shifts and assignments for Resident Care Associates. The Wellness Coordinator will also be assigned caregiving shifts as needed to provide coverage or support.

  • Effectively manage call outs, time off requests and other issues with the schedule, finding appropriate coverage per state requirements and community guidelines
  • Provide RCA coverage as needed if openings in the schedule cannot be filled
  • Manage and file schedule changes and related documents, such as approved requests for vacation, personal, or sick time
  •  As directed by the RCD, support the interviewing, on-boarding and ongoing training of new Resident Care Associates for both Assisted Living and Compass Memory Support
  •  As requested, provide administrative support to the RCD such as updating assignment sheets, supply lists, new move-in paperwork, gathering licenses or certification documentation, etc.
  •  Guide RCA’s in their duties, assigning them to cover Assisted Living or Compass Memory Support Neighborhood floors; ensure that Assisted Living and Compass Memory Support Neighborhood floors are covered at all times during associate meal and rest breaks
  • As directed by the RCD, review task sheets and medication sheets daily at end of shift to
    ensure proper documentation

REQUIREMENTS:

  • Strong preference for candidates who are certified as a Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) with experience in geriatric care
  • Desire and ability to work well with seniors with a compassionate, patient and understanding manner, including individuals with Alzheimer’s disease and related dementia
  • Strong technology skills with ability to learn systems SLR uses: Payright/Kronos (scheduling and payroll software), Google Apps for Work (Gmail, Drive, Docs, Sheets, Slides), Google calendar management, web-based project management. Working knowledge of general office equipment
  • Strong time management and communication skills
  • Must be able to read, write, speak and understand English
  • Leadership skills with the ability to work effectively with others in a team environment and promote a culture of teamwork, cooperation, enthusiasm, customer service, and an atmosphere conducive to growth for all associates
  • Must be able to handle the physical requirements of the job including standing, sitting, bending, and moving and transferring residents (one-person or two-person assist), as well as physical ability to lift up to 50 pounds as required
