When a resident makes Compass on the Bay their home, we become their partner in healthcare.
In alignment with the wellness and support services your loved one receives at our community, we maintain an open dialogue with your healthcare providers to gain a full understanding of their care needs. If you are looking for a healthcare provider, we bring in several local practitioners directly to the community. We are also happy to recommend outside practices in the Boston area. Whether you are already a resident at Compass on the Bay or are transitioning to your new home, we have you covered with visiting physicians, mental health services, PT/OT, and more.
Resident choice is always at the forefront of our care planning, and you have complete control over the healthcare providers in your network. When you move into Compass on the Bay, you can explore working with our onsite partners or maintain relationships with your current providers.
Onsite Care Partners
Primary Care Coordination
We work with Gather Health to offer onsite primary care, bringing a highly skilled team right to our door! With Gather, residents have access to healthcare professionals with extensive experience in senior care. Direct-to-Community Pharmacy Coordination
We partner with Greater Boston Long Term Care Pharmacy, who provide traditional pharmacy services such as medication packaging and delivery directly to Compass on the Bay. With this partnership, residents have the worry-free and convenient process of getting their medications on time, right to their doorsteps.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you have conditions that require assessment, rehabilitation and recovery and would like to see providers directly in the community, we bring in local practices so that you can return to your normal activities as soon as possible.

Outside Care Coordination
If your loved one needs to leave the community for additional care, you can rely on us to support you every step of the way. We maintain constant communication between residents, families, and the hospital or rehab center, and our Wellness team will work with Case Managers to play an active role in the discharge process.
Our top priority is to help your loved one return home to Compass on the Bay. We will work to review any updates to their care plan and ensure access to additional services should they need them.
