Alongside the wellness and support services your loved one receives in our community, we work closely with healthcare providers to ensure a well-rounded understanding of their care needs. For those searching for a healthcare provider, we partner with local professionals who conveniently offer onsite services, and we’re happy to refer you to other practices in the greater Malden area.
Whether you are already a resident at Forestdale Park or are transitioning to your new home, know that we’ve got you covered, from in-house primary care services to partnerships with local care providers like physical therapy, psychiatry, vision, dental and more.
Resident choice is at the forefront of our care coordination. The decision of who to include in your healthcare provider network is entirely yours. When you make the move to Forestdale Park, you can continue seeing the providers you know and trust or choose to work with our onsite providers.
Onsite Care Partners
Primary Care Coordination, Visiting Physicians, Geriatric Care Management
For residents who prefer convenient care in the comfort of their apartments, we bring in practitioners from Vantage Healthcare twice a week. With Vantage, residents have access to healthcare professionals with extensive experience in senior care.We also work with Vesta Healthcare, who offers onsite primary care as needed.
Residents seeking a Geriatric Care Manager have the option of working with Lifecare Advocates. Their services include:
- An evaluation of physical, cognitive and emotional health
- Recommendations for services that can be accessed locally
- Ongoing support and monitoring
- Consultations, healthcare advocacy and long-term care insurance support
Mental Health & Wellness Services
We know that our residents’ emotional health is just as important as their physical health. If you need extra support to work through challenges and changes in life, you can choose to work with Thriveworks Counseling & Psychiatry Cambridge, which offers both in-person and online therapy. Thriveworks features a team of Licensed Mental Health Counselors, Clinical Social Workers and Marriage & Family Therapists, and they accept most insurance.
Direct-to-Community Pharmacy Coordination
Through our partnership with Greater Boston Long Term Care Pharmacy, you don’t have to worry about traveling for prescription pickup! We coordinate medication deliveries directly to Forestdale Park, and residents can also choose to receive additional clinical support, such as detailed drug reviews and assessments.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you have conditions that require assessment, rehabilitation and recovery and would like the option of receiving care in the community, we partner with local practices so that you can return to your normal activities as soon as possible.
Vision, Dental, Hearing & Podiatry Providers
Other specialists in our network of care provide additional services such as dental care and podiatry, some of which are offered onsite at our community.

Outside Care Coordination
We are fortunate to be located in an area with some of the nation’s best hospitals and healthcare facilities. If a resident needs to leave our community for a hospital or rehab stay, our nursing team stays in touch every step of the way to make sure we are ready for their return.
Our shared goal is to help your loved one return home to Forestdale Park. Together, we’ll review any necessary changes to their care plan and explore additional services to provide the support they need.
