Cornerstone at Hampton offers two living options on one campus: Independent & Assisted Living and Compass Memory Support. Our experience with seniors and their families have taught us that you don’t want to move just because you need some extra assistance, so our care options are meant to help residents age in place as their needs change over time. The primary goal is to preserve seniors’ independence and dignity and give them the best possible quality of life. Before moving in, our Nurse assesses each resident and develops a personalized wellness care plan, created in collaboration with our team, their family, and physician. This plan is tailored to reflect individual needs, preferences, and goals, and is reviewed and updated twice a year, or as needed.
One monthly rate covers:
- Nurse Assessment & development of Individual Service Plan
- Certified Resident Care Associates (CNAs) available 24/7
- Onsite Wellness Nurse
- Personal care assistance with daily activities and tasks
- Maintenance & utilities for your apartment
- Light housekeeping and flat linen laundry
- Three meals daily in our restaurant dining room (featuring Brain Healthy selections)
- EnrichedLIFE experiences & adventures
- Scheduled transportation services in the community limobus
Additional Care Services Provided Onsite
Cornerstone at Hampton works with a variety of trusted local care providers to offer additional support beyond each resident’s community wellness plan, allowing you to conveniently receive PT, OT, speech therapy and related services without leaving our community.
We have strong relationships with local healthcare providers who visit residents regularly, including:
- Local Physicians and Nurse Practitioners who conduct on-site “house calls” and visit weekly
- Temporary Agency Skilled Nursing (VNAs) which can provide all sorts of medical care on-site such as post-surgical wound care
- Podiatry, and Physical, Occupational and Speech Therapies
You are welcome to maintain your relationships with your own physicians, and will help to arrange transportation to your doctors’ visits so that you don’t have to switch providers.
